• Select ‘Unknown’ in the accessibility advisor field if you are a new student who has not been assigned an advisor yet.
• Affix an electronic scanned copy of your document. Maximum size: 30MB.

*If the PDF contains images of the document, these must be legible on a white background without any shadows in a single PDF file.
